Federal workplace‑safety officials have opened an investigation after an employee died during what company leadership described as a “serious incident” at the Candle‑Lite manufacturing plant in Leesburg.

According to Luminex President Michael Callahan, the incident occurred Thursday and resulted in the death of a team member. Emergency crews responded immediately, and the company stated it is cooperating with OSHA and other authorities as they review the circumstances surrounding the fatality.

Callahan said the company’s focus is on supporting employees and that no further details about the incident would be released at this time out of respect for the ongoing investigation.

The Occupational Safety and Health Administration confirmed that a formal investigation is underway. OSHA investigations can take up to six months to complete, and the agency does not release additional information until the process is finished. Neither OSHA nor Luminex publicly identified the employee.
